Store refresh rates in millihertz and preserve three decimal places.
Advertise exact rational rates while accepting legacy integer values.
Accept rates from 23.900 through 1000.000 Hz.

Rebuild the driver's cached mode list before replugging the monitor so
Windows enumerates the settings saved by the helper.
Keep saved mode refresh rates intact and update only ExtraMode to the
configured default refresh.

Following the example of CPipeClient in the helper, this switches to using
overlapped I/O for easy interruption and cancellation that doesn't quite
work with CancelSynchronousIO.

As the IDD itself runs in a WUMDF sandbox, it doesn't have enough
access to perform interactive operations such as moving the cursor.
This helper service communicates with the IDD over a named pipe,
so that we can perform these things, as well as in the future provide
a configuration GUI.
